Welcome to the Isle Care learning disability community services page
Our goal is to support people with learning disabilities have a fulfilling, enjoyable, and healthy life. We aim to support people to take as much control of their lives as possible, ensure they can make the same choices we all do, good and bad, and experience the same emotions we all experience, happy and sad. How do we do this?
We strive to include the people we support in all we do, such as developing policies, procedures and practices. Some people we support have helped us produce some DVDs on how to complain and on selecting support staff; others have helped us produce an introductory DVD called our support provider.
Every person we support has an individual person centred plan. This details what support is needed and where the support is coming from. This support might be from support workers, families, day centres, and health practitioners such as dentists, doctors, and occupational therapists. The plan is designed around the person and outlines what they need, want and desire. It is their plan.

The team operates across the Southern Region and support adults with learning disabilities. Staff support people with individualised support packages in their own homes, shared housing, or accommodation based Supporting People services, across the Southern Region. The highly motivated, experienced and qualified team aims to maximise quality of life for the people it supports.
The team work to the Governments ‘Valuing People’ agenda and in close partnership with local authorities. The team prides itself on innovative and high quality work, built around person centered planning, integrated with the identified 6 keys stages (need to link to 6 keys to citizenship) leading to real citizenship for people with learning disabilities.
